Chronic Care Management (CCM) is a Medicare program where clinical staff help manage a patient’s chronic conditions through a comprehensive care plan that can include education, outreach, assistance with medication, appointments and refills, and other between-office-visit support.
CCM was established in 2015 by Medicare and is billable. In 2022, Medicare increased the average payment for its primary code (99490) by 56% to $64.02 per patient per month.
